[R6RS] Refining `violation'

Now that we've settled on the condition hierarchy, it seems it makes
sense to either add a procedure `contract-violation' that does the
same as the current `violation', but uses &contract instead of
&violation.  (Actually, I think, in light of this, `violation' should
then be ditched.)  Objections?
