They were used a lot before we switched to case-sensitivity, and I
think they're rarely used now. Many people used to insist on writing
case-sensitive code, but no one seems to insist on writing
case-insensitive code: I see a lot of now-redundant `#cs's still in our
sources (nearly always as a module prefix) and no `#ci's.
I doubt that adding these prefixes in R6RS would create many problems,
but I agree that the appendix could be a better place.
